TEMPORARY/EXTERNAL CARDIAC PACING What is Temporary Pacing? A temporary method to support patients with brady- and tachyarrhythmias until a permanent pacemaker is implanted. It involves delivering electrical impulses to the heart via transvenous or epicardial leads connected to an external pulse generator. Indications: ✅ Severe bradycardia or heart block with hemodynamic compromise ✅ Post-myocardial infarction conduction disturbances ✅ Drug-induced bradycardia ✅ Electrolyte imbalances ✅ Overdrive pacing to suppress tachyarrhythmias (e.g., VT or Torsades de Pointes) Types of Temporary Pacing: - Transvenous pacing (via subclavian vein) - Epicardial pacing (post-cardiac surgery) Key Parameters to Master: - Rate: Determines how often the pacer fires - Output: Energy delivered to the lead - Sensitivity: Current needed to detect intrinsic activity - Mode: Controls lead pacing and response to native activity (e.g., VVI, DDD) Credit: @OnePagerICU
