2) I am puzzled so repeated marrow 7 months later but looks same! Pt still refuses therapy. Pt just turned 96 & doing amazingly. What is longest period you observed newly diagnosed #AMLsm with no therapy and with pt doing well with Mike cytopenias & no need for transfusion?

1) Patient in their mid 90s but excellent PS,mild cytopenias but no PB blasts, marrow shows #AMLsm with 40% blasts, normal karyotype, NGS with 2 DDX41, NRAS, SRSF2, SETBP1 mutations. Refuses therapy as feels well with good counts, I see monthly but nothing changes for 7 months
