Had a convo with 8 Black men last night (1 was unvaccinated and 4 of us are docs). The unvaccinated brother asked a very serious question about the vaccine containing a tracking device. None of us responded with a judgmental response. He's considering getting the vaccine now...

TOMORROW (Dec 30) having significant oral surgery (sinus lift/bone graft, bridge cut, tooth extracted & implant) husband & son to keep eye on me as i recover. honestly, pain is more in bank account. crazy how no insurance covers this - non-cosmetic procedure.

Incredible memories of being part of an inaugural program training the next generation of Kaiser Permanente family physicians. Despite the cracks and 1970s decor, that place served us well.

@hope411adcock @Bela_rocks I’m generally strict with opiates because I’ve witnessed really poor outcomes with liberal prescribing and polypharmacy. Every year I find myself more careful—goal for me is as a last resort and smallest dose possible. But a “no opiate” policy lacks all common sense. I hear you.

@ConnectTheDocs @Bela_rocks You might be surprised at how many *would* argue that. Their opinion is that a No Opioids Policy is the best policy. Individualized Patient Centered Care isn't an option in their world view😐

Before an injection or surgery, there is always informed consent with a patient. But prescribing a narcotic is actually much more dangerous than most surgeries in terms of poor outcomes. #KPSD18

In the end, it’s always an “N of 1: the patient in front of you.” EBM is about trends and new research in medicine, what generally work/fails, and makes us rethink strategies as physicians. I try to learn from both evidence and my patients. Heck, sometimes Twitter ;)

I hate telling patients over 65 that muscle relaxers are not ideal, but more and more research points to cutting down and eventually getting people off zanaflex, baclofen and similar. Prescription yoga,🧘‍♀️ bath bombs, massages and heating pads ! #Jama @KPSanDiego

I almost quit med school as an MS2. Stopped studying. Suddenly bombed a bunch of tests as a result. Didn’t even register for Step 1. After a long talk with my husband, I had a change of heart on the last day to register. Med school sucks. Don’t let anyone tell you otherwise.

@marklewismd Yup. Told by a few patients I was a murderer and going to hell for not prescribing it. BUT most patients consistently still thanking me for being there for them every day.
