Iran's ballistic missiles were launched from deep underground facilities carved into granite mountains, too hard for U.S. aircraft to destroy.
The Pentagon settled for collapsing the entrances while conserving bunker busters for China and North Korea contingencies.
Iran has since excavated many back into operation.
Iranian commanders, possibly assisted by Russian intelligence, studied U.S. flight patterns and exploited their predictability.

UPDATE: Three people were shot and killed outside a San Diego mosque, including a security guard who may have stopped an even bigger massacre.
Police say the two suspected shooters, ages 17 and 19, were later found dead in a car near the mosque from apparent self-inflicted gunshot wounds.
Investigators reportedly found anti-Islamic writings in the car, a suicide note, and one of the guns used in the attack had the words “hate speech” written on it.
