That road is Route 1 in Iceland. A week driving it costs roughly $2,500 per person. Flights from the US run $500-600 round trip. Gas is $8-9 per gallon. A glacier hike is $125. A night in a decent hotel near Vatnajökull is $160-200. Total tab for two people to spend a week staring at that glacier instead of a monitor: somewhere around $7,000. The median American household earns that in about 18 working days. Sitting in a room. Staring at a screen. The people who actually drive that road on a random Tuesday in March fall into two categories: retirees who stared at screens for 40 years and saved enough to stop, or remote workers who figured out how to stare at a screen from Reykjavik instead of a cubicle in Ohio. Both paths run through the screen. The photo is real. The freedom it represents costs $7,000 and 10 days of PTO. The device you’re reading this complaint on is the same device that books the flight.

@kevinmd On call providers don't charge for services rendered when called in? Don't disagree there should be compensation, of some amount, particularly if no reimbursementfor services provided. But I've seen many a doc hold hospitals hostage for exorbitant on call rates of $1500+/night.

I interviewed an internal medicine physician who says the medical system relies on a massive amount of unpaid labor to function. We call it "taking call." Dr. Corinne Sundar Rao joined me to discuss why the traditional model of physician on-call compensation is a primary, yet rarely discussed, driver of burnout. For decades, taking call was simply baked into the job. You worked your full clinic day, you were on standby all night for the hospital, and then you worked a full schedule again the next day. It was justified by the "calling" of medicine. But as Dr. Rao points out, the complexity and volume of modern medicine make this model unsustainable. Other high-stakes professions, like commercial airline pilots, have federally mandated rest periods. Yet surgeons and physicians are routinely expected to make life-and-death decisions on zero sleep, often for little or no extra pay. Dr. Rao argues that "call" is a euphemism for extracting free labor from physicians to cover the hospital's unassigned patients. We have seen successful solutions before. The hospitalist and laborist models proved that we can turn endless, tethered responsibility into defined, compensated shifts. But many specialties are still trapped in the old paradigm. The result? Physicians aren't complaining; they are simply disappearing. They are dropping out of traditional practice, moving to concierge models, or leaving medicine entirely. If hospitals want to solve the staffing shortage, they need to stop relying on altruism to subsidize their 24/7 operations. Call is labor. It isn’t racist to choose not to watch Bad Bunny. I don’t care what language he sings in or his ethnicity, as a mom of 3 young men, I don’t support exposing them to men dancing around in dresses. That’s a moral decision as a Christian, and I’m allowed to set that boundary for my family. It doesn’t make me hateful or racist. I’ve turned off halftime shows before—like Lady Gaga and Rihanna—for depictions of the devil and blatant blasphemy. I respect what others choose to watch and don’t hurl insults or slurs. But as for me and my house, we will serve the Lord.

@MySportsUpdate There are three Pro Bowl QBs. One is out, currently, with the Super Bowl. 2 are maybe. If those 3 are 1-3, and next three are 4-6 (debatable, but, still),... what about QBs 7-14 for the AFC? 'Cause I'm pretty sure Shadeur is 16, maybe 15...

The AFC’s Pro Bowl QBs were Drake Maye, Josh Allen, and Justin Herbert. Maye is going to the Super Bowl. The other two are banged up. We'll see if they stay. Bo Nix got injured. Patrick Mahomes is injured. Daniel Jones is injured. That’s how it trickles down to Shedeur Sanders.

One thing many patients don’t quite realize is what actually happens in a doctor’s mind during a consultation. For most male doctors, especially when examining female patients, there’s an almost automatic switch that flips off. Not consciously. Not with effort. It just happens. Any sexual framing disappears completely. The person in front of us stops being “a woman” in that sense and becomes simply a patient. A history to take. A problem to solve. A human being who needs care. Clinical mode takes over. And honestly, nothing else survives that shift. It’s less complicated than people imagine.

CAN A MALE DOCTOR ATTEND TO A WOMAN IN LABOUR ??? A few years ago, I was on duty in the evening (night call), when this pregnant patient walk in. Labour had started in the afternoon. Both she and her husband came together. However, shortly after their arrival, kasala burst o. The husband was asking where the female doctor was. At the moment, there was no female doctor on duty. He started shouting that his wife deceived him that we have female doctors. Even if we do, did you pay them to be on duty whenever your wife fall in labour? How was anyone to know that your wife would be in labour that day? Anyway, the man became very angry, and screaming out his lungs at his wife The woman was not taking it o. She started her own shout; "You this man, you're very w!cked. Your wife is in labour but all you're concerned with is not having a male doctor attend to her. You want to k!ll me so you'd marry another wife. I'll not go anywhere with you i cannot d!e and leave my children, I'll deliver in this hospital..." No be small thing o. My own concern was that, they should resolve their matter let me know whether to attend to her or not, since I was the doctor on call, who would likely supervise her delivery. And it should be written and signed that no matter what happens, I should not interfere. I communicated this clearly. Eventually, after plenty talk from the MD and the man's wife, he received a little sense and said I can attend to his wife. I ensured I minimised my visit to the labour room. Around 1am, she delivered and sustained cervical tear. I was called to repair it because she was bleeding. We had to lock the labour room while I was there, just incase the dev!l enters him, he then decide to make trouble again. So, before you become choosy on the kind of doctor you want to attend your wife's delivery, find out if the hospital have such capacity. Also note that Nigeria don't have enough doctors to afford you that privileged of deciding who should or should not manage your wife's labour and delivery. There's absolutely nothing wrong with a male doctor attending to a woman in labour. However, some people have biases towards a male doctor conducting or supervising delivery. So, if you're such person, ensure you sort it out with the management before registering your wife for antenatal so you don't end up embarrassing yourself and constituting a nuisance in the hospital.
