News flash: LSVT LOUD defeated NHS speech and language therapy for Parkinson's disease dysarthria (unclear, slow, slurred speech) in a new @bmj_latest randomized clinical trial by Sackley and colleagues. Key Points: - The authors tested LSVT LOUD vs NHS speech and language therapy vs control. - It was pragmatic, multicenter, three arm, parallel group, unblinded, randomized and controlled. - 388 folks enrolled. - Self-reported voice handicap index used as primary outcome measure. - The LSVT LOUD group had a lower voice handicap index score (improved). - Most common adverse event was vocal strain. - Folks followed for a year w/ benefit (intervention was only a few months). My take: I am so glad to see data driven studies comparing different approaches to treatment of dysarthria in Parkinson's disease. We focus so much on drug development, however it is just as important we test Parkinson's behavioral and non-pharmacological interventions. Though more costly and associated with potential vocal strain, LSVT LOUD should be considered as an evidence-based option to address dysarthria in Parkinson’s disease. The authors felt that the vocal strain was treatable when it occured. One key issue will be developing person specific treatment plans for the boosters or re-interventions, to be sure the effect is maintained. bmj.com/content/386/bm… #parkinson #speechlanguagepathologist #SLP @FixelInstitute @ParkinsonDotOrg
