I have attended the 5th ATLS student provider course at @sgrhindia as a faculty for 3 days (19/11/25-21/11/25) and I'm afraid I have absolutely killed it. #ATLS #Trauma

@Bappieidk You get it! That's because more and more anesthesiologists are included in ATLS 11th ed. Before that, it was mostly surgeons.

@DocPeriop ATLS 10th ed mention Drug asst. Intubation but its like "some non day to day airway manager" read up an excerpt on RSI and slap it there. 11th ed is way more nuanced

Congratulations, you've encountered a case of classic boomer. That said, ATLS 10th ed doesn't really talk about drug assisted intubations or physiologically difficult airways. Some instructors have started teaching 11th ed but 10th ed is still the basis for taking exams. The transition is happening live. You feel it, don't ya! What you do in the Trauma bay is all that matters and I know you'd be doing it right🕺✨️

I retired as Medical Superintendent of Sevagram Hospital three years ago. Since then, I have been waging a quiet war against forgetting. It began with blank stares. A first-year resident, fresh from her MBBS in Kerala, drew a blank when I mentioned Dr. Sushila Nayar, our founder. Another asked, “Dhirubhai who?” about a former president. These were the people who built MGIMS. Yet their names are fading in the very place they shaped. I have called this project Architects of MGIMS. Institutions are not made of bricks and reports alone. They are shaped by those who arrive, argue, disagree, create, persist—and slowly leave their mark. To piece this together, I went back to annual reports from the 1970s, faculty records, and long conversations with retired teachers now in their eighties—their spouses, children, and colleagues. I met those still on campus. So far, I have written 93 profiles—not a polished history, but something of the texture of our early years, before silence claims them. It now lives as a digital archive, open to all. We begin today, on Id-Ul-Fitr, with the first profile—Dr. Sushila Nayar. ATLS talks about physiologically difficult airway and drug assisted intubation. Yet, during practice session of the provider course, the instructor was pissed at me for putting iv and asking BP before intubation🫩and he told me that I'll fail. Nevertheless, got certified.🙂‍↕️

Autism: “The spectrum has become so accommodating that I fear that it has now been stretched so far that it has become meaningless and is no longer useful as a medical diagnosis.” Problem is that psychologists, and increasingly a flood of master’s level therapists have come to rely on the autism diagnosis to support their practice. Their livelihood. I suspect schools face a similar reliance on autism diagnoses for extra funding. Furthermore: Parents love it because their child gets extra services. Students love it because it makes them special. Drug companies love it because they profit from drugs. Psychologists are supposed to be the gatekeepers on diagnoses, but we are not doing very well at it.

@ArunBee Agree. If it's not good enough for you to eat, especially if it has got potential to make you sick, you are not being generous giving it away to someone who can neither afford to take a day off or good health care.

The guys who hand their IndiGo sandwich to their driver because they don’t like it are often the same ones who buy a dozen bananas, eat eight over a couple of days and on the third day generously “gift” the remaining, almost-rotting ones to their maid. Generosity… with an expiry date. 🍌

Appreciate you putting your efforts into speaking about such topics sir. But I feel the narratives can definitely be less vulgar. The line between casual style-speaking a language your audience understands and normalizing insensible and indecent exchange of words is blurred. You are a writer, I'm sure you can find alternate ways of writing, keeping it sensible and parliamentary.

ఇరగదీత అప్పటివరకూ ఎన్నో నీలి చిత్రాలు చూసి నలిపేసుకున్న నాగరాజుకి పెళ్లయింది. రెండో ముహూర్తం ఇంకో రెండ్రోజుల్లో. ఎప్పుడెప్పుడా అని ఎదురు చూస్తున్న రాత్రి రానే వచ్చింది. ఇంట్లో వాళ్ళందర్నీ ఇంకో ఇంటికి వెళ్లిపోమన్నాడు నాగరాజు. సిగ్గుపడుతున్న పెళ్లికూతురు సరోజ మనసులోనే “ఈ రాత్రికి ఇరగదీస్తాడు అయితే” అని అనుకుంది. మొత్తానికి పాలు పట్టుకొచ్చిన ఆమెని మురిపాలు ముచ్చట ఏమీ లేకుండా వివస్త్ర ని చేసి గట్టిగా పట్టి నలిపేస్తున్నాడు. సరసం అంటే ఇలాంటి యుద్ధమే ఏమో అనుకుని నొప్పి, తీపిల మధ్య సరోజ మౌనంగా ఉండిపోయింది. నాగరాజు ఆగలేకపోయాడు, గబగబా పంచె విప్పేసి పైకొచ్చాడు. సరోజ కింద పడుకుని అతనికోసం ఎదురు చూస్తోంది. ఎందుకో మనిషి ఆగిపోయాడు. ఆమె ఎందుకా అని చూసింది, అతను ఆమె చెయ్యి పట్టి లేపి నాలుగుకాళ్ల మీద ఒంగోమన్నాడు. ఆమెకి సిగ్గనిపించింది. మిషనరీ లోనే చేద్దాం అని అడిగింది. అదేంటి అని అడిగాడు నాగరాజు అయోమయంగా. ఆమె వివరించింది. కుదరదు నేను ఎన్నో నీలి చిత్రాల్లో చూసాను, వెనకనుంచి చేద్దాం అన్నాడు. ఆమె డాగీ వద్దంది. ఇతను వినలేదు. ఆమె వెన్నక్కెళ్లి నిల్చున్నాడు. ఆమె “మెల్లగా” అంటూ అతనిది పట్టుకుని దారి చూపబోయుంది. ఆమె చెయ్యిని విసిరి కొట్టి “నాకు తెలుసులే” అని ఒక్క ఉదుటున లోపలికెళ్లడానికి ప్రయత్నించాడు. అంతే “అమ్మా” అని అరుపు. ఆ అరుపు సరోజది కాదు, రెండు కాళ్ల మధ్య చేతులు పట్టుకుని, ఒంగున్న సరోజ పక్కన పైకి సీలింగ్ ఫ్యాను చూస్తూ పడున్నాడు నాగరాజు. “అయ్యో చూడనీ” అని అతని చేతులు తీసింది. అతని మగతనం మధ్యలోకి వంగి, ఉబ్బెత్తుగా అయింది. అదేంటో ఆమెకి అర్థం కాలేదు కానీ ఇదేదో తేడా అనుకుంది. ఇంట్లోవాళ్లు వచ్చి మంచం మీదనుంచి, కార్లో పడుకోబెట్టి డాక్టరు దగ్గరికి పట్టుకెళ్లారు. పరీక్ష చేసిన డాక్టరు, నాగరాజు పెనిస్ విరిగింది అని చెప్పాడు. “విరగడానికి అదేమన్నా ఎముకా! కర్రా!” అని నాగరాజు నాన్న జైయ్యని లేచాడు. “చూడండి, మీవాడు ఏదో అలవాటులేని పొజిషన్ ట్రై చేసి ఉంటాడు, సరాసరి ఆవిడ పెల్విక్ బోనుకి కొట్టేసాడు, ఎంత గట్టి మగతనం అయినా విరిగిపోద్ది.” అని ముక్తసరిగా చెప్పాడు. “ఈ ఎదవకి ఇంకా ఒక పొజిషనూ తెలీదు, నువ్వేమో పొజిషన్లు అంటావు!” అనుమానంగా అడిగాడు నాగరాజు వాళ్ల నాన్న. “మీ వాడికి థియరీ ఫుల్లు, ప్రాక్టికల్సు నిల్లు. అదీ విషయం” అంటూ నోట్సు రాస్తున్నాడు డాక్టరు. నాగరాజుని ఒకటి పీకాడు వాళ్ల నాన్న. “ పడకగదిలో అతను వయోలెన్స్, క్లినిక్లో మీరు వయోలెన్సా, అసలు మీ ఇద్దరికీ వయోలిన్ నేర్పాలి, సున్నితత్వం అంటే ఎంతో తెలుస్తుంది.” అన్నాడు డాక్టరు పైకి లేచి. ఇప్పుడేం చెయ్యాలి అన్నట్లు నాగరాజు, వాళ్ళ నాన్న, సరోజ అందరూ ముఖం పెట్టారు. “చూడండి, పెనిస్ లో పైన రెండు ట్యూబులు ఉంటాయి. కింద ఒక ట్యూబు ఉంటుంది. కింద ట్యూబులోంచి మూత్రం వెళ్లే నాళం వెళ్తుంటుంది. ఇక పై రెండు ట్యూబుల్లో మూడు వచ్చినపుడు అందులోకి రక్తం వెళ్లి అవి గట్టిపడి పెనిస్ నిలబడుతుంది. ఇప్పుడు ఇతను పెట్టేటప్పుడు కొత్తదనం, చీకటి వీటన్నిటి మధ్య కన్ఫ్యూస్ అయ్యి ఆమె మానం ముందు ఉండే ప్యూబిస్ అనే ఎముకకి గట్టిగా తగలడం వలన ఒకవైపు ట్యూబు విరిగిపోయింది. అందులోంచి రక్తం లీకయ్యి అది ఉబ్బిపోయింది. ఇప్పుడు నేను ఆపరేషన్ చేసి దానిని రిపేరు చెయ్యాలి, ఇంకో రెండు వారాలు బ్యాటింగ్ కుదరదు”అని వివరంగా చెప్పాడు డాక్టరు. ఇరగదీస్తాడు అంటే ఇరగ్గొట్టుకున్నాడా అనుకుంది సరోజ. “మళ్లీ ఇలా అవ్వకుండా ఉండాలంటే!?” అని అనుమానంగా అడిగాడు నాగరాజు. “ మీ ఆవిడ చెప్పింది విను, ఎక్కడ ఎలా పెట్టాలో ఆమెకి తెలుసు, నువ్వు ఆవేశపడిపోకు. అలాగే బ్లూ ఫిల్ములో పొజిషన్లు ట్రై చెయ్యకు, వాళ్ల లాంటి కండలు గానీ, వాళ్ల బ్యాలెన్సు గానీ నీకు లేవు. అలాగే సెక్సు అనేది నువ్వేదో నిరూపించడానికి కాదు, మీ ఇద్దరూ కలిసి ఆనందించడానికి.” అని చెప్పి నాగరాజుని థియేటరుకి తీసుకెళ్లాడు డాక్టరు.
