But there are so many Mabels. After the third discharge plan for Mabel has failed, a manager asks the ward doctor: "How frail is Mabel? Is she in the last 6 months of life?" "Possibly..." "Has anyone spoken to her about all her options?" "What do you mean?" "Well, has anyone asked if she is suffering unbearably? It's important she knows all the options open to her. It's important she has real choice." That's how this will go. Little conversations like this. Throughout the NHS. For every Mabel.

Very proud & privileged to have been a part of this. Huge energy & potential in the room to tackle the issues, support the NHS as key part of the primary care team, and find ways of working /future models to support patients further. Look forward to what we can achieve together.

This would make so much sense, save NHS time and resource and really support patients. Pharmacists train 5yrs to become experts in medicines so sensible, safe substitutions is completely within our scope.
