Supine PCNL Tip Instead of relying only on the posterior axillary line, the tip of the 12th rib can serve as a simple and reliable landmark for the initial puncture. A point ~2 cm below and slightly posterior to the 12th rib tip works well as a starting point. The trajectory can then be adjusted depending on the targeted calyx.
