Injury to BIG BLUE?! 🔵😨 First, take a deep breath🫁 Then, perform a R ➡️ L medial visceral rotation, use sponge sticks to compress the IVC against the spine, & reassess the situation. Get access, blood 🩸, & help 🆘! To repair, "define the edges" (as they say in TOP KNIFE) & place a clamp or start suturing. You can try multiple Allis clamps placed in sequence along the laceration, removing clamps as you sew. Or, a Satinsky clamp👇🏼 If the patient is in extremis, the infrarenal IVC can be ligated with reasonable outcomes.
