Very grateful for the opportunity to speak at this year's @EM_Update hosted by @NYGH_News I highly recommend EMU. Incredible content, incredible speakers. I've summarized my talk below, 5 high impact interventions/considerations outside of the trauma centre.

Levophed if need some more support especially if neuorogenic deficits.

RSI consider lower dose of ketamine 0.5-.75 mg/kg or etomidate 10-15 mg. Document neuro exam before intubation (pupils, GCS, lateralizing signs). TXA 2 g bolus in 1st 15 min. No cryoprecipate if you can get a level but give if you can’t and bleeding. 1-3 g calcium.

Dr Andrew Petrosoniak at %nyghemu2026: blood/MTP for anyone bleeding with a hemodynamic derangement (prehospital hypotension, altered LOC, POCUS, SI>1, RABT => 2, ABC =>2). Balance perfusion vs hemostasis ~sBP 95-105.
