Body mass index and breast cancer risk among postmenopausal women with and without cardiometabolic diseases: Findings from two prospective cohort studies in Europe

In a pooled analysis of over 168,000 postmenopausal women from the EPIC and UK Biobank cohorts, higher BMI was significantly associated with increased #BreastCancer risk, particularly among those with pre-existing cardiovascular disease (CVD). While BMI was linked to breast cancer risk across the board, the association was notably stronger in women with CVD (HR 1.31) compared to those without (HR 1.13), suggesting a synergistic effect (p_interaction = .02). No such interaction was observed for type 2 diabetes (T2D).
The combination of overweight/obesity and CVD was associated with an excess of 1.53 breast cancer cases per 1000 person-years beyond expected rates, highlighting the importance of cardiovascular health in modifying obesity-related breast cancer risk.
