#OTNearField on #EnsiteX made this post-maze AFL a breeze- and work of art! Majority of the circuit emphasized at frequencies >450hz with perfect entrainment within. Isthmus in RSPV made for a signal buffet & 2s termination. Thanks to Dr. Riley and Dr. Tan at HUP for this beauty!

As if the signals didn’t speak for themselves, our OT vectors beautifully illustrated the wavefront’s complex navigation around the PV anastomosis and our last deflection algorithm made sure no late mid-diastolics were left behind! Thanks for a fantastic case, @CerantolaMaxime !

@mike_lean @maddyferraro1 @Dr_Santangeli With those slow, late potentials in sinus and the full cycle length at site of termination, we suspect it was microreentrant. The OT vectors seem to suggest a circuit as well!

@maddyferraro1 @Dr_Santangeli @EnglanderLily Interesting one. What do you suspect the mechanism of the tachycardia is, at the border of presumably isolated veins? In both sinus & tach, split potentials seen along most of Grid, with fractionated bridging potentials on just a few bipoles. Is it small, local reentry?

Redo longstanding persistent AF. Dissociated AF in the LA posterior floor following CS isolation. In some patients the arrhythmogenic portion of the posterior wall extends way below the usual LA posterior box segment in between the PVs @natale_md @AhmedBAlsalem @PennMedicine
