For most people, getting Covid was a disruption that passed. But for the millions who developed #LongCovid, that disruption has been persistent – for years. After 32 months of living with this complex chronic illness, here are some reflections...🧵

We need a moon shot to understand and treat #longcovid - a disease projected to impact as many people as heart disease in the next decade. This understanding requires team science and could alter how we think about the immune system for future pandemics nature.com/articles/d4158…

We often talk about being at "some percent" of previous function in #LongCovid #MECFS Lets get a dataset together: % Physical % Cognitive % Social/Interactive % ADLs - basic activities of daily living I would be: 10/5/15/25 100% would be previous health @remissionbiome

@RenegadeRes - loving your work. If #mecfs and #longcovid are systemic, could be a cascade of causes -e.g. initial trigger ->inflammation ->dysautonomia -> #sibo/ #pots -> social isolation -> etc. Combinations of treatment may be needed to address all maintaining factors?

Great update from Dr Norman Swan on #LongCovid in Australia. In addition to eye-opening numbers (400k in Vic alone), research about the pathway between Covid, inflammation and mental ill-health discussed. Along with the need for coordinated care. abc.net.au/listen/program…

Stories of the personal impact of #MECFS #LongCovid are heartbreaking. When our lives suddenly stop - skills, knowledge and capacity are also suddenly lost from our workplaces and economies. Investing in effective treatments and supports is essential to stop the #lostCVsofME

@TraceySpicer Thanks for sharing - it can feel so strange suddenly not being able to do what you used to do so easily. Vertigo and crashes in general are nasty!

A few thoughts on #LongCOVID, crashing and vertigo. Moral of the story: If you have a chronic or invisible illness, listen to your body. 🙏🏻

@trishgreenhalgh From personal experience, it has to be an obscure art-house film that seems endless, is all soft-filtered shots from weird angles and you can't follow the plot of!

@CoyneoftheRealm As a clinical psychologist with a research PhD - now with a living experience of #MECFS - loving the posts @CoyneoftheRealm no amount of reframing will cure PEM. Much more helpful to focus on disconnection from networks, changes in identity, frustration, loss and FOMO!

Citizen-scientists in the #MECFS and #LongCovid community need to appreciate the CBT offered for #MUS and functional somatic symptom disorders (#FSSDs) is not garden-variety CBT and there are high rates of deterioration in both the control and intervention groups, only less in the intervention group. The “CBT” did not map well on international understandings of the assumptions and delivery of CBT. The complex intervention included weeks of indoctrination of the patient with an understanding of their physical problems that incorporated simplistic pseudoscience before any CBT was delivered. The CBT offered in this trial focused on goals imposed by a psychiatrist that didn’t necessarily fit with patients’ sense of their most pressing problems and solutions. Among the takeaway messages in my recent @Substack article. …mcoyneakacoyneoftherealm.substack.com/p/danish-rct-o…

Service design - such an important element of effective care - but sometimes easy to miss when we focus on intervention level research. Great to see this conversation at #SMHR2022 @ian_hickie
