It's called plastic surgery. In the picture on the right, his nose has the same look and shape. His eyebrow meat looks remarkably similar, especially the unique scar tissue, folds, or whatever he has going on just over his left inner eyebrow. Look at the crows feet at the end of his right eye, and look at the forehead wrinkles - they bend upward like he's had some tightening/lifting done. Also, his flesh just forward of his ear has a curved line that looks like where he might have had tissue pulled to tighten up is skin. Biden was a terrible President, but the idea that it was some kind of body double is a reach.
