#WPW Left-sided AP? Consider CS catheter via internal jugular vein. Femoral approach often limits distal reach along the mitral annulus. An eccentric CS activation suggests a left-sided pathway, but true localization comes from CS bracketing.

Simple but interesting case to finish the year: redo inferoseptal WPW. The EP study started with no pre-excitation at all—normal HV interval, with no pre-excitation during RA or CS pacing. The only way to unmask pre-excitation and map the pathway was through sensed PACs from the CS. How often do you see this? @ALFIEEP1 @LAHRSonline1 @narrowQRS
