Kris is very clearly their own character (not a self insert), and everyone in the game uses they/them for them. Also, Kris has both a doctor and a nurse outfit for the "Take Care" act.

Number 1 concept for a fetish game is forever just a farming sim where you fatten the entire town. You obviously give fattening gifts to your love interest so they'll date and marry you, but you can optionally gift everyone else too. 10 heart stages? Instead 10 portrait/sprites.
