John Sterling is as much a part of the fabric of the New York Yankees organization as any player. It was an honor to have him call every one of my games during my career and an absolute pleasure to get to know him on a personal basis. My thoughts and prayers are with his family and friends. He will be deeply missed.

Viral Outbreak at Sea?
A Netherlands-based cruise ship (@OceanwideExp) with around 150 passengers fell victim to a suspected hantavirus outbreak.
As of right now there are 3 reported deaths, and 3 illnesses (one in the ICU in South Africa).
Carried by rodents (droppings), this virus can result in severe respiratory or kidney-related illnesses when contracted. When the lungs are affected, known as hantavirus pulmonary syndrome (HPS), it proves fatal in around 38% of cases.
The @WHO is currently investigating and hantavirus has been confirmed in one of the six victims thus far. There are no drugs considered as treatment, supportive care being the typical course of action.
Evacuation efforts are underway.

Development of the BAVENO VI criteria will allow patients with cirrhosis to safely avoid endoscopies!
The criteria involve non-invasive tests that have proven effective in ruling out significant portal hypertension (SPH) and high-risk varices.
Ruling these two specific issues out will help reduce unnecessary screening by 21%.

Increased Heart Failure Rates Among Younger Patients
Between 2010 and 2022, hospitalization for heart failure in adults <65 years old has increased, while rates in elderly decrease.
For the younger patients this reflects an increase in cardiometabolic risks, while the older populations trend is likely due to increased professional management.
Increasing youth obesity, hypertension and diabetes can play a role in these findings.

Anorectal Morbidity Noted After Bariatric Surgeries
A Saudi Arabian study led by Hefzi Alratrout used a self-reported questionnaire to help link bariatric surgeries to benign anorectal symptoms.
Though these symptoms are often not serious, they can certainly hinder a patient's quality of life.
The two major connections that were found?
1. Laparoscopic sleeve gastrectomy - constipation.
2. Laparoscopic Roux-en-Y Gastric Bypass - diarrhea.

A recent study performed out in Greece suggests we need FAR better post-translational surveillance of infection, as well as better prevention methods.
The study involved patients over the age of 50, all who have undergone varying transplant procedures.
These include liver, kidney, heart, pancreas, double organ transplants.
The results were that 59.6% of these patients had contracted a Klebsiella spp. infection within 180 days of the transplant surgery.
This study determined two main risk factors, each contributing two-fold to the increase in infection likelihood.
1. Patients with a history of CVD (cardiovascular disease).
2. Patients whose post-operational stay was 3+ days.
These two factors alone played a large role in infection likelihood, and the results themselves show an urgent need for infection prevention among these patients.

REAL PATIENT EXPERIENCE - Allergic Reaction Call🚑
Our crew was called to the scene of a local bank for a male employee in his early 20's who was having an allergic reaction to peanuts.
Arriving on scene: Local PD was set up outside; Fire had already made contact and was bringing the patient out on the stretcher. The allergy was new to the patient, and he had mistakenly consumed peanuts while on break at work.
Signs/Symptoms:
- scratchy/itchy throat
- swollen throat and lips
- wheezing and coughing
- reddening of lips and face
Vital Signs:
- BP: 132/78 (normal)
- HR: 117 (tachycardic)
- RR: 18 (normal)
- SpO2: 92% on room air
- BGL: Normal reading.
- EKG: Normal reading.
Interventions:
- Patient self-administered Benadryl prior to our arrival.
- Epinephrine administered IM
- Albuterol/nebulizer treatment given.
- Solumedrol (steroid) administered.
Funnily enough I have a tree-nut allergy as well and being only a year older than the patient I took it as a way to relate.
I told him about my most recent anaphylactic reaction, I gave him some tips to avoid nuts or determine if a product was okay to eat.
We laughed a bit, and in what was likely the scariest moment of his life, he was smiling. That's what healthcare and emergency medicine is all about.

The @US_FDA has granted preapproval access to an investigational new drug produced by @RevMedicines
Daraxonrasib, a RAS(ON) inhibitor, is now accessible for patients with previously treated metastatic pancreatic ductal adenocarcinoma (PDAC).
For now, this availability only warrants use in life-threatening situations (or clinical trials), when alternatives have been exhausted. This can be done under an Expanded Access Protocol (EAP), though the drug looks to become accessible to all PDAC patients in the future.
Daraxonrasib has shown to improve survival rates of patients with PDAC to 13.2 months, compared to 6.7 months with standard chemotherapy.
Revolution Medicines has done great work, aiming for safe and equitable patient access.

Pancreatic "Cyborg" Islet Cells are being developed!
Using flexible electronics to monitor and enhance the maturation of lab-grown pancreatic cells, looking to massively improve diabetic therapies going forward.
Islet Cells, known as the islets of Langerhans, are responsible for producing the major pancreatic hormones that help regulate our blood glucose level (BGL)!
Without these hormones, specifically insulin and glucagon, it can lead to systemic dysregulation - hyper or hypoglycemia. These conditions, stemming from either high or low BGL can have severe impacts on other organ systems, notably the brain.
These cyborg cells could then be transplanted into a dysfunctional pancreas, allowing natural production of the hormone to start up again.
This could look to broaden treatment options, especially offering hope to those with Type 1 Diabetes - an autoimmune disorder causing nonfunctional pancreatic beta cells (type of Islet cell).
