2023 has undoubtedly been the most dreadful year of my life. As it draws to a close I’m on the brink of having to say goodbye to a dear friend who I’ve just been told may not make Christmas. How i have found the strength to carry on this year is quite beyond me.

@ChurchfieldJE @CornwallCouncil @BBCCornwall This is the Discharge to Assess model. Aimed at reducing over prescription of care from a hospital bed. Current approach causing over use of care home beds and denying residents opportunity of returning home.

Good morning #Cornwall Care packages in Cornwall are now to be arranged AFTER people are discharged back home from hospital. @Foweydoc who’s in charge of Adult Social Care @CornwallCouncil is with me on @BBCCornwall today to tell you how it will work.

@ChurchfieldJE @CornwallCouncil @BBCCornwall To clarify the change - long term care need will be assessed after discharge in resident’s homes, not hospital. Of course support will be put in place initially focused on rehabilitation whilst longer term needs are assessed.
