I've seen too many older adults "diagnosed" with #dementia and continued indefinitely on antipsychotics (sometimes for years!!) because it wasn't clearly documented on discharge the patient had acute #delirium! @DukeGeriatrics @AgingDuke

I thought I could "tell" when a patient had delirium...then I started using standardized tools (3D-CAM). Now I recommend it to anyone who wants to do better! Accurately recognizing #delirium is important!! #thisisgeriatrics
