Last week I saw in OPD someone who in my opinion needs to start infusional 5-fluorouracil as soon as possible. I do not have access to a phlebotomy service during my clinic. Luckily for the patient, a highly competent nurse volunteered to take the blood sample for DPD testing. I filled out the two separate request forms required to ask for one portacath insertion Today I learned that the earliest portacath insertion appointment was in 3 weeks time. This is because the lists between now and 3 weeks from now are already fully booked. My options are to spend the most part of an hour tomorrow, calling around asking for portacath insertion favours, or to try to find a way to admit the patient to hospital, in the hope that portacath insertion and treatment delivery might happen sooner. Three weeks is too long in this case, in my opinion. In this whole example, I can assure you, that the majority of the HSE employees involved, including hospital management, in my experience, are routinely going above and beyond their job descriptions to try to help out, to bend the rules, to add to an already full procedure list, to compromise available resources to get the job done. This type of case happens most days of the week. The capacity of the system is a critical limiting feature.
