Today we observe World Adherence Day – a powerful reminder that treatment only works when it is followed consistently. 🌍💊 Non-communicable diseases (NCDs) such as obesity, diabetes, hypertension, heart disease, stroke, and chronic kidney disease are increasing rapidly across the world, especially in developing countries. The challenge is no longer just diagnosis — it is long-term adherence to treatment and lifestyle changes. Why adherence matters: • Patients with hypertension who regularly take medicines can prevent heart attack, stroke, and kidney failure • Proper adherence in diabetes reduces complications such as neuropathy, retinopathy, and cardiovascular disease • Sustained lifestyle adherence (diet, physical activity, sleep, weight management) is the most powerful treatment for obesity • Patients with chronic diseases need lifelong care — not short-term treatment The real problem is not lack of medicines. NHL Municipal Medical College this past Friday. My session, "Obesity & Circadian Rhythm," delved into how our modern "24/7" lifestyle often clashes with our internal biological clocks, leading to significant metabolic consequences. We discussed how chronobiology isn't just a sub-speciality—it is the fourth dimension of precision healthcare. A few key takeaways from the discussion: Timing is everything: When we eat and sleep is just as critical to metabolic health as what we eat. Circadian Disruption: Persistent misalignment of our internal clock is a hidden driver of the obesity epidemic. Clinical Integration: We must start integrating circadian biology into standard clinical practice to improve patient outcomes. Prediabetes, Cardiac Injury, and Heart Failure Risk for Adults With Hypertension: A recent JAMA Cardiology (Jan 2026) study highlights an important and often under-recognized risk group: adults with hypertension and prediabetes who also have subclinical myocardial injury or stress. Using data from the SPRINT trial (8,234 adults with hypertension but no diabetes or prior heart failure), researchers examined whether prediabetes combined with cardiac biomarkers predicts future heart failure (HF). Prediabetes was defined by fasting glucose 100–125 mg/dL, while cardiac injury or stress was assessed using high-sensitivity troponin I (hs-cTnI) and NT-proBNP. Key findings: Nearly 40% of participants had prediabetes. Over ~3 years of follow-up, HF risk was markedly higher when prediabetes coexisted with elevated cardiac biomarkers. Compared with normoglycemia and no injury: Prediabetes + myocardial injury → ~4-fold higher HF risk Prediabetes + myocardial stress → ~5-fold higher HF risk Prediabetes + either injury or stress → >10-fold higher HF risk Importantly, prediabetes alone (without biomarker elevation) was not strongly associated with HF. Longitudinal increases (≥25%) in hs-cTnI or NT-proBNP over 12 months further amplified HF risk, especially in those with prediabetes. Why this matters: Prediabetes appears to act as a metabolic amplifier, magnifying the impact of early myocardial injury or stress rather than serving as an isolated HF risk factor. This study supports moving beyond glucose levels alone. Clinical takeaway: In hypertensive patients, combining glycemic status with cardiac biomarker profiling may substantially improve heart failure risk stratification and enable earlier, precision-based prevention—even before diabetes or overt symptoms develop. #prediabetes#heartfailure#hypertension#myocardialstress#myocardialinjury jamanetwork.com/journals/jamac…
