@GalloniJonathan I agree but he was very determent about running. interestingly he did have a CMR from one year ago (which I just saw) in a different center. he had transmural infarct at the time but he was advised to continue to run 50km

@HaticeKGunsel In Italy in a case like this we would not permit competitive physical activicty, even after ICD implantation (see Eriksen). We might let him do non competitive activicty though

@GalloniJonathan Coronary angiography was stent previous stent from ACS patent, no new lesions. I would recommend ICD too..

@HaticeKGunsel Now that you are explaining the case, everything makes sense. Answering your question, first of all I would take a look at his coronaries. I think that most likely there would not be a progression. I would start OMT since EF is reduced and then if no PCI i would go for ICD.

@GalloniJonathan thank you for the comment. I have slightly different opinion: he is an Athlete so the enlargement of cavities is cardiac adaptation, he has a history of acute coronary syndrome one year ago and the basal inferolateral transmural infarct is the cause of VT. Would you let him run?

@danimilovanovi1 @Interve15538505 Yes that is true CMR is usually bot needed, but this patient was referred for right heart enlargement thinking it might be ARVC or PAH. so I think its important to look out of the box in such cases to be able to see different diagnosis.

@Interve15538505 @HaticeKGunsel Scimitar syndrom. A type of PAPVC where right PVs drain into IVC. Often diagnosed in CXR due to a curved sword-like appearance (scimitar = turkish sword) and eventually confirmed with echicardiography. CMR is usually not needed
