Injections into the bursa can facilitate rehab. Calcium, if present, can be washed out with a needle and rarely requires an operation. Cortisone injections into the bursa can work well in some cases... PRP might work well for tendinopathy that doesn't respond to rehabilitation alone. Fun day .. “You’re my 5th opinion.” Knee injury These are usually a problem. Previous 4… -Only one exam -All repeated X-rays -2 MRIs. - One Special NY hospital often insists their MRIs are better and wants new ones 🤦‍♂️. The fourth doc started PT 🙌 🥳 4 surgical recommendations... but 3 different procedures. PT is working. Me: “Do you think you need an operation?” Them: No. I feel pretty good. Me again. “Cool… nice meeting you. Have a great weekend”. I wish these were rare or isolated cases.

@hjluks Absolutely agree but imposible to explain to mayority of patients when they read the radiologist inform, even more, when they visit a PT

Most shoulder pain = Have shoulders +/- menopause +/- type 2 diabetes (or IR), +/- thyroid disease, or any combination thereof. Your MRI result doesn't matter nearly as much as you think it does. Every adult has cuff findings and labral tears.

@northwoods1980 CS are like aspirin and morphine. They are so efective and cheap that everyone is talking trush of them. But agree w you. If you can go for an arthroplasty, no sense repeated shoots.

Call me simple, but I’ve never fully understood the logic of chasing short-term results. For example, using steroid injections in an arthritic joint when the patient already knows they’ll eventually need a replacement. I can see the value if there’s a specific short-term goal like getting through a big event or an active summer but I doubt that’s the case in most scenarios. If me, I wouldn’t want steroids injected into my body I’d rather just have the joint replacement and move on with life. What am I missing? How often do you actually see a patient become asymptomatic after a steroid injection and enjoy years of relief? From what I’ve seen, the literature shows worse outcomes in patients who undergo multiple injections before eventually having arthroplasty. On top of that, steroids can harm bone health and have other neg physiologic effects. It feels like I’m missing something important here.

@Lentejitas El mensaje es acertado aunque un poco confuso en su redacción. La SS lleva un retraso considerable en el control de la incapacidad temporal lo que hace que muchos casos pasen automáticamente a incapacidad permanente independientemente de su estado de salud.

Y todo así: El desbordamiento de la incapacidad temporal de más de dos años dispara a un millón los casos permanentes.
