@benshapiro Can I ask a genuine question as a Brit who just doesn't understand? Why do so many people in the US want to own a gun? Never have I ever wanted to have one and I'm wondering is it culture, fear or something else entirely? I'm not trying to be annoying but just want to understand

When anyone suggests measures designed to prevent school shootings that AREN'T large-scale gun control, the media mock them. Even if those measures would be far more effective and achievable than Democrats' largely-unspecified policies. Which means this is all posturing.

I'm an NHS dietitian. In typical headgear from the 2020/21 #ICU collection, and current 2021/22 style. I'm vaxxed x3 because witnessing the vagaries of severe #COVID19 for the past 22 months in 100's of #ICU patients up close has been, frankly, terribly sad. Just #GetVaccinated

Just a reminder that if you're prescribing Pabrinex I+II, Parentrovite or B co-strong for your patient with or without a macrocytosis, check B12 and folate levels and supplement accordingly. None of these B group vitamin supplements contain B12 or folate. Major oversight

NG feeding does not suppress appetite.. I repeat 📢 NG FEEDING DOES NOT SUPPRESS APPETITE. Please stop removing feeding tubes when nutritional intake is suboptimal. We need to make sure nutritional intake is optimal to aid with rehabilitation and recovery @ICUnutrition #SOA21

@daisyrichens @Carys06 @ICUnutrition I'd generally suggest all SLTs and dietitians in a department know each other really well so communication is tip top and issues can be dealt with quickly. I find reliance on others to portray messages sometimes leads to issues.

@HollyMondayJon1 @Carys06 @ICUnutrition Of course, and I hope it doesn’t seem as though I’d make any decisions re nutrition myself! My initial q was whether me suggesting that *medics/nurses liaise with dietetics* re altering NG feed with a view to stimulating appetite was a misguided one (which is evidently the case)!

@daisyrichens @Carys06 @ICUnutrition Sometimes I do trial a reduction in feed when other things have been ruled out. I'm really quick to review though after decisions like that to make sure I can bump it back up if it doesn't go to plan. It can be sensible in some cases though so do keep liaising with us 👍

@HollyMondayJon1 @Carys06 @ICUnutrition And my subsequent questions were curiosity re what else would/could be done *at the recommendation of dietitians* (tho I’ll admit that wasn’t clear). Would never dream of NOT referring to/liaising with dietitians, just want to make sure I’ve not got a totally wrong understanding!

@daisyrichens @Carys06 @ICUnutrition I think non DTs recommend these things because they're generally aren't enough DTs, especially in critical care and acute settings. For reassurance you aren't necessarily wrong to think these things but ref to DTs would ensure safe changes that could be more easily monitored 🙂

@daisyrichens @Carys06 @ICUnutrition Sorry to jump on this but I found this really interesting. For me, it comes down to scope. I personally work really closely with SLT but I would always refer any swallowing issues to them not make recommendations myself if that makes sense.
