Homelander has been like that since season 1, and even earlier than that in the original comic book (where, among other things, he stages a coup and successfully overthrows the US government), which ended in 2012.
The problem is Trump behaves like a deranged comic book villain.

The Boys is another example of the Hollywood left trying to villainize the right and make them look awful. Similar to what they attempted with the Starship Troopers movie. They have increasingly tried to make Homelander into Trump, with him flipping out over memes, comparing himself to Caesar, etc. Where they failed is that everyone still likes him. Regardless of what he does, he still has fans. I know it drives the producers insane. It's little things like that, that make me happy.

I genuinely don’t understand how some person who works for an insurance company can have the ability to decline treatment or deem “not medically necessary” when it is prescribed or ordered by a licensed medical professional.
The whole system is unethical.
