Distal ACA bifurcation aneurysms are among the most technically demanding lesions in neuroendovascular therapy. This report highlights successful WEB treatment of a broad-neck A4 bifurcation aneurysm in a bihemispheric ACA while preserving both distal branches without stenting.

Interesting study on an “in-hospital stroke bypass” system for AIS patients requiring MT who were not initially identified through the regional stroke network. Direct MT team activation by trained EMTs, bypassing emergency physicians, significantly reduced D2P/D2R times.

Medial deviation of the ICA (C2) may be associated with ACP/OS overdevelopment. This can create an acute ICA–OphA angle, leading to poor visualization of the OphA and technical difficulty. In this setting, the OphA may arise medially—easily missed if assuming a superior origin.

A bailout technique for FD deployment with a distal aneurysm. Releasing Surpass Evolve within the DAC, proximally withdrawing the delivery wire, and then unsheathing the DAC helped avoid distal wire migration. Wire behavior management is also important in FD procedures.
