@MaxJordan_N Kcl crystals exist as a salt substitute (brand name NoSalt) that isn’t that bad actually! Haven’t tried the KCl we give to patients for repletion though.

Hey Medicine, Renal, Cards friends: when we aggressively diurese ppl with lasix/diuril and chase their potassium, replete incessantly etc, I find myself wondering… why don’t we use a little spironolactone? Can someone explain why we dump & replete vs adjuncting a K-sparing med?

Thoughts of an IM resident on an off service EM block: 1) Holy wow EM providers are seeing a stunning volume of patients. I have no idea how they juggle so many patients at such variable stages in their workups and in acuity. It makes my head spin to try and keep up.

@MaxJordan_N Not sure what the experience is like at BWH, but at least over here, “email is a second medical record”. It’s common to send emails to outpt providers of hospitalized pts. I found it strange, but not sure if there’s a better way. I don’t know why we don’t use inbasket this way.

It is too logistically hard to donate blood in a city that is ridden with hospitals. I work in a major medical center and the closest @RedCross site is a good 15-20 min drive away with traffic and they stay calling my phone. There’s gotta be a better way 😐

Health infrastructure so crazy that we have specialists whose job is to seemingly make sure we write notes with specific diagnoses so that hospitals can bill in a certain way and squeeze more $ out of insurers, whose goal to to hold tight onto & amass money (aka try not 2 pay?)😵‍💫

@ChristleNwora It’s so hard to pay full attention and learn the same way on zoom lectures. (Tangentially, I often feel v privileged that throughout this whole year I still got to come to work in person and see my colleagues, while so many of my friends had to work isolated in home offices 🥺)

The ability to attend in person conference has dramatically improved my educational experience in residency. I attended in person classes all thru medical school and wasn’t a fan of recorded lectures. I love the ability to commune with others and stay focused on the material.

We defined contrast induced nephropathy, as a creatinine increase of 0.5 mg/dl or 25% within 48 hours of exposure. If we defined Flozin Induced Nephropathy similarly, these drugs would be a major nephrotoxin.
