@DrBenLovell Important to also recognise that no one looks great or capable in hospital gown or pyjamas and in bed, get people up and dressed in their own clothes and they suddenly become themselves again! Hair done, teeth in, hearing aid and glasses on...it's magic 🥳

They facilitated a five way conversation between the Frailty virtual ward, the Frailty Team and a paramedic who was in the house with the patient and their family to develop a shared plan and kept the patient at home 🥳 #teamwork
