@ToughLoveMD Many docs have a similar realization as they consider a “medical director” role. The lack of credentials, training, and experience at some of these med spas is horrifying. This situation is tragic but at least it’s bringing some awareness.

When I was seeking "easy" supplemental income as an ER doc, I signed with an agency matching physician medical directors with NPs and RNs running med and IV therapy spas. The money? Lots of it for little effort. I did not know then the ethical and legal Faustian quagmire I had stepped into. I started reviewing spa owner applications to work under my license and was horrified by the lack of oversight, basic physiological knowledge, or understanding of current legislation. Some of the NP spa owners/franchisees had literally never worked clinically, as the majority of their education was online. Some had never put in an IV on "real patients" - only mannequins. Safety protocols? Minimal. Patient screening? A check box form detailing vague risks and hoping that no one lies about their medical history (spoiler alert: most people historically do to get their magic potions). Physician oversight? Most supervising physicians simply hold a medical license in the state where the spa operates and never set foot on-site. Malpractice offered? Ridiculously scant for oversight of medical treatments. Within a few months, with not a single patient treated under my license, I ended my contract with zero confidence that any of the NPs scrambling to open a clinic would maintain a standard of care that met my own. For decades, I have worked with (and continue to work with) incredible NPs and PAs in the ER - hell, I was a PA-C myself before medical school. But, if you are seeking a quick fix, be an educated consumer - better yet, eat a whole food diet and drink some f-cking water. It's cheaper, safer, and won't potentially kill you.

🚨FROM SELF-CARE TO TRAGEDY: The Fatal Reality of the IV Spa Trend Just twenty-four hours after posting a glowing review online about a local Texas medspa praising it for making clients "look and feel beautiful" a 47-year-old mother’s life came to a sudden, tragic end. Jenifer Cleveland went in for what she thought was a routine wellness treatment. She never returned home. 💔 The Deadly "Cocktail According to the Texas Medical Board, Cleveland was administered an IV containing a potent mix of vitamins and electrolytes, including a heavy Total Parenteral Nutrition (TPN) solution containing concentrated potassium chloride. The solution was reportedly infused far too quickly, triggering sudden cardiac arrest in a matter of minutes. What makes this case truly horrifying is the total lack of professional oversight uncovered by investigators: Unlicensed Administration: The spa owner, Amber Johnson, was not legally licensed to perform the procedure. No Medical Presence: Not a single trained medical professional was on site when the infusion was administered. A Distant Director: The clinic’s registered medical director was reportedly over 100 miles away. Zero Safety Net: There were no real medical safety protocols or emergency response plans in place to handle a crisis. When Cleveland went into distress, the staff lacked the training to save her. Despite emergency CPR and a rush to the hospital, she was pronounced dead shortly after arrival. Amber Libhart Johnson (Med Spa Owner) Amber Johnson, the owner of Luxe Med Spa who allegedly administered the IV treatment without a valid medical license, turned herself into the Freestone County Jail on April 28, 2026. She is facing 14 criminal charges, including: Felony murder Manslaughter Criminally negligent homicide Delivery of a dangerous drug (9 counts) Tampering or fabricating physical evidence Practicing medicine without a license Johnson has since posted a $69,000 bond. Dr. Michael Patrick Gallagher (Medical Director) Dr. Michael Gallagher served as the spa's medical director. He was also indicted by the grand jury and turned himself in to authorities on April 29, 2026. According to the Texas Medical Board, Gallagher failed to properly supervise the clinic and allowed an unlicensed individual to administer prescription solutions and dangerous drugs, resulting in severe complications that the staff was not trained to handle. ⚠️ A Critical Warning for the Public Boutique IV bars are opening on every corner, aggressively marketing energy, weight loss, and hydration cures. But this case proves that behind the luxury aesthetics, some of these operations are cutting life-or-death corners. This wasn’t just a simple mistake. This was an unlicensed business playing doctor with fatal consequences. Ask yourself: Would you ever let someone connect an IV to your bloodstream without verifying their credentials first?

Humility is a diagnostic tool. Knowing the limits of your own knowledge is what keeps patients safe and keeps medicine moving forward.

Physicians are under more pressure than ever. From administrators, from insurers, from legislatures, from a public that increasingly questions our expertise. Nobody is going to advocate for us if we don't advocate for each other.

@ALMannixMD Absolutely. Ironically, saying “I don't know, but I'm going to find out” seems to build more trust with a patient than pretending to have an immediate answer.

I thought being a good doctor meant having all the answers. It actually means knowing when you don’t, and what to do next.

When trust in healthcare breaks down, physicians get lumped together with insurance companies and pharma. I get it. The system has failed people. But your doctor is usually fighting that system too.

Distrust of doctors is rampant. Patients willingly trust an influencer with an affiliate code over a physician with 14 years of training. And we are letting it happen by staying quiet.
