Adding a second-sided MRI-guided focused ultrasound subthalamotomy for Parkinson's. Some important new data was just published @JAMANeuro from Raúl Martínez-Fernández, Jose Obeso and their team from Spain. Time to ab uno disce omnes. Key Points: - The authors asked whether adding a staged FUS-STN second contralateral lesion was safe and effective. - The motor improvement was statistically good and the adverse events 'mostly mild and transient.' - There were only 6 people in this series and each had been treated with unilateral FUS-STN 'contralateral to their most affected body side, and whose parkinsonism on the untreated side was described as progressing and was not optimally controlled with medication.' - The original cohort had 45 people previously treated and 7 lost to follow-up; 4 excluded due to adverse events. - Of the remaining 34 patients, 6 experienced 'progression of parkinsonism on the untreated body side and this is how they got to the small number treated. - Following the second FUS-STN lesion, 4 of the 6 had new 'contralateral choreic dyskinesia, which resolved by 3 months.' - 4 people had speech disturbances described as gradually improving but persistent in 2. - 1 person had mild imbalance and dysphagia during the first week after treatment and it was described as resolved by 3 months. - No behavioral or cognitive disturbances. My take: Bravo to the authors for publishing their data and sharing what happened to the 6 people in an original cohort of 45 who later progressed in their Parkinson's and received a second sided lesion. I love the careful approach of staging a second lesion, and also of monitoring and reporting on outcomes we most worry about. Historically, second sided lesions have been associated with pseudobulbar, speech and cognitive side effects. It will be up to the new generation of scientists to explain why we saw those effects (in the past) and if in the future, we carefully add a second sided brain lesion, how to avoid them. Let's go slow and steady and understand lesion locations relative to potentially important fiber bundles and let's understand sizes of lesions and how these impact outcomes. One of the big worries for STN lesions is the hemiballism or dyskinesia, and it was nice to see it resolve in all of their cases (4/6). Will this become a persistent problem for a few folks who have this procedure and will it require an additional GPi DBS or pallidotomy if persistent? Too early to know, however it will be important to discuss in shared decision making for all those considering a staged (delayed) second lesion.
