As a resident I didn’t know if I would be interested in hospital medicine because one of the things I loved most about medicine was the relationships. I was afraid I would lose that as a hospitalist because I wouldn’t have continuity or long term patients. 1/

@TheSurajKannan I think you’ll find a lot of attendings disagree and appreciate EBM/value based care. If you articulate rationale and present step wise testing approach, that’s best IMHO. Anyone can send tests, more impressive if you’re judicious.

General observation about residency - you will almost always appear more intelligent/thoughtful/cautious/whatever if you order the workup than if you don't. Feels harder to develop a minimalist approach until you're an attending, presumably by which time habits have set in...

Trump has nominated Dr. Oz as the Centers for Medicare & Medicaid Services (CMS), it's critical to prioritize expertise, integrity, and a commitment to the most vulnerable populations in healthcare. (1/x) axios.com/2024/11/19/tru…

@DrDiGiorgio No physician led practice would allow patients after hours help / advice? Whether it’s MyChart or something different, the expectations from patients of physicians is different as are our work loads.

This is an easy fix. Restore physician independence and ownership. Of course doctors want to clock in and clock out when they are employees of a large faceless health system. They have no ownership. Care is led by administrators. They feel it is just their job to be data entry clerks, upcoding notes and answering CDI queries to maximize revenue. Show up, glue yourself to the computer for 12 hours, leave... Restore physician independence and ownership and that problem goes away. There are policy solutions to restore physician independence. All our professional societies and organizations need to back these policy efforts: Site Neutrality 340B Reform Regulatory burden reduction Quality metric reform Reversing the ban on physician owned hospitals Stark law reform Then we can get back to physician-led care. @anish_koka @DutchRojas @mass_marion @elonmusk

@BenMazer Don’t forget over half the newer generation of physicians are female. Either by choice or necessity, they have other “work” outside of work.

@acweyand Also, I can consider medicine a calling/profession and not want to sacrifice my personal health or those of my loved ones for it. We signed up to be healers, not martyrs.

One final sunrise before a final sunset today. In the cruelest way possible, while my father lay ill, my mother, by his side every day, suffered a hemorrhagic stroke. Know life is so fragile, and if my mom has taught me anything, it’s to be kind and assure people are fed 😔

Watching @DanMatasic try to keep a straight face as I dropped facts about aspiration PNA in the CVPCU (with fairy wings in place) was a highlight 😂

It’s the end of the year for academic medicine and start of a new one. It was a privilege to work with some amazing future ophthalmologists and stellar crew of @OslerResidency residents this week on Brancati service along with @allenkachalia . Can’t wait to meet the new interns!

Got stuck in an elevator for 10 minutes while on my way to morning teaching rounds…what a way to start a Monday! Hopefully the rest of the week is smooth sailing from here 🤞🏻#MedTwitter

Today was my last day of @OslerResidency! Grateful for the faculty, peers, and patients who inspire me and taught me so much over the past 7 years at Hopkins. Also honored to have received the Charlie Wiener Award for Education! Next stop: @SinaiCards Fellowship 🫀
