@TheSurajKannan I think you’ll find a lot of attendings disagree and appreciate EBM/value based care. If you articulate rationale and present step wise testing approach, that’s best IMHO. Anyone can send tests, more impressive if you’re judicious.

@DrDiGiorgio No physician led practice would allow patients after hours help / advice? Whether it’s MyChart or something different, the expectations from patients of physicians is different as are our work loads.

This is an easy fix.
Restore physician independence and ownership.
Of course doctors want to clock in and clock out when they are employees of a large faceless health system. They have no ownership. Care is led by administrators. They feel it is just their job to be data entry clerks, upcoding notes and answering CDI queries to maximize revenue. Show up, glue yourself to the computer for 12 hours, leave...
Restore physician independence and ownership and that problem goes away.
There are policy solutions to restore physician independence. All our professional societies and organizations need to back these policy efforts:
Site Neutrality
340B Reform
Regulatory burden reduction
Quality metric reform
Reversing the ban on physician owned hospitals
Stark law reform
Then we can get back to physician-led care.

@BenMazer Don’t forget over half the newer generation of physicians are female. Either by choice or necessity, they have other “work” outside of work.

@acweyand Also, I can consider medicine a calling/profession and not want to sacrifice my personal health or those of my loved ones for it. We signed up to be healers, not martyrs.

Grateful to the @WSJ and @tepingchen for drawing attention to this issue. It is possible to be a dedicated physician and still want work-life balance. They are not mutually exclusive. Glad to work at a place that recognizes that (@HopkinsMedicine)
