Should we prescribe physical therapy after STN DBS for Parkinson's? YES. Since there is limited data on this subject Matteo Guidetti and colleagues performed a Delphi expert consensus on the topic and the results are not surprising. Key Points: - Limited low-quality knowledge on the subject in the published literature. - Panelists agreed PT could improve motor symptoms and quality of life post-DBS. - PT is a safe intervention. - Well-designed and well-performed clinical trials (e.g., blinded RCT) could "provide higher-level evidence for PT post DBS." My take: Physical therapy has a well established evidence base in Parkinson's disease. It is unfortunately, under-utilized and under-prescribed. The guidelines support its use in Parkinson's and it would make no sense to not prescribe it post-DBS therapy. Most folks who receive DBS have motor fluctuations and are at potential risk to fall, especially if two DBS leads are placed. We should, in my mind, be proactive and prescribe PT before and after DBS. Even better is to plan to have the physical therapist checking in regularly, to proactively prevent falls and fractures.
