I used to think this too until I re-read the books. The movies failed to capture Ron’s intelligence and charm. Hermione, an only child from a muggle family, would be drawn to the large, magical Weasley family. Therefore legally and ethically she must be FULLY informed before any procedure is performed on her body including the pros and potential risks, and she must SIGN a consent form before a tubal ligation.

I get how tough this is for her, she's already carrying so much risk with her body worn out from all those pregnancies. As the doctor, I'd quietly respect her direct request and do the tubal ligation during the C-section since her life is genuinely on the line, then explain things clearly to both of them afterward. Saving her from a possible deadly bleed matters more than anything else right now.

She is 38 years old, currently in her 10th pregnancy. Her previous 9 deliveries were all successful, but her body is tired. Her uterus is like an overstretched elastic band at high risk for Uterine Atony and fatal Postpartum Haemorrhage (PPH). 🩸📉 As the Doctor, you have a private conversation with her. She’s exhausted.
