Severe acute limb ischemia in patients with COVID-19: a single-center case series
🚨As a CV surgeon, this study pushed me to a quick and wider ALI literature review:
COVID-19 is STILL silently amputating limbs, even in vaccinated patients.
AGAIN, COVID-19 isn’t “just a respiratory” disease!
➡️Review results:
Systematic reviews confirm ALI as a hypercoagulable complication of COVID-19:
- 191 patients across 56 studies in a 2023 review sciencedirect.com/science/articl… and,
- 55 cases in 35 papers in earlier reviews, plus
- multiple case series including this original Chinese 7-patient Rutherford III cohort!
‼️So, this is certainly NOT a rare Covid-19 complication.
➡️Patient profile:
- Predominantly male (70-80 %), mean age 50-65 years, but younger is possible,
- Common comorbidities include hypertension (~50 %) and diabetes,
- Occurs in severe COVID-19 (30-40 %) but also in mild/asymptomatic (10-15 %) or even delayed in post-recovery cases (10-20%),
➡️Presentation & labs:
- Mostly lower-limb (80-85 %), popliteal artery most affected,
- Younger patients (<60) showed markedly faster onset,
- Pain, cyanosis,
- Variable Rutherford grades (IIa–III),
- Frequent multi-vessel or aortic involvement,
- Ischemic features: lower-limb ulceration, gangrene, often bilateral,
- Near-universal elevated D-dimer (90-100%),
- Inflammatory surge signs included neutrophilia, lymphopenia, markedly elevated D-dimer, fibrinogen, hs-CRP, and PCT, along with hypoalbuminemia and often raised cardiac markers (BNP, LDH, cTnI),
➡️Pathogenesis:
COVID-driven endothelial dysfunction, cytokine storm, and immunothrombosis cause macro- and microvascular occlusion, even in vaccinated/post-Omicron patients,
➡️Management & outcomes:
- Anticoagulation and surgical thromboembolectomy are first choice,
- Conservative therapy or advanced flaps/debridement used in severe necrosis,
- Amputation rates 10-30%,
- Mortality 20-40 % (often from respiratory failure), far worse than non-COVID ALI.
‼️In conclusion, let there be no doubt, COVID-19-associated acute limb ischemia remains a devastating prothrombotic complication carrying persistently high amputation and mortality rates despite modern interventions, serving as a grim reminder of poor overall prognosis, fully inline with the findings of present study!
‼️So, I’ll say it again:
Prevention beats every scalpel, every amputation, every coffin.
Don’t get infected,
Mask, ventilate, stay out of crowds,
Get up to date with your vaccines,
Your legs and your life aren’t worth the risk.

My first covid infection 2021. I was 16. I was so unwell. Noticed this rash so took a photo. Now I’m severely chronically ill with multiple conditions and I still notice this rash flare up sometimes. I wish someone had warned me. #longcovid #mcas

@AineAine1984 I guess that could relate to some people, especially those with kids etc. but in my case I live alone and didn’t see no one apart from my work colleagues. I caught covid from them and now severely disabled as a result

@LCPerspectives There was no way of telling if people got covid in work from a colleague/patient or outside of work in the supermarket. HCWs were provided with FFP2/3 masks that do in fact protect you from covid positive patients. Underlying conditions contribute to lasting effects from viruses

In the EU #Covid injury is recognised within the workplace.
In Ireland and UK 🇬🇧it is not recognised or compensated like HIV, or Hepatitis C
