In this original research study, 61% of patients in the Enhanced #PrimaryCare Diabetes program achieved all five diabetes care goals (D5). However, Black patients were significantly less likely than White patients to reach these goals, despite having more nurse interactions. annfammed.org/content/23/4/3… These results point to persistent inequities in diabetes outcomes and underscore the need to tailor care models to better support all patient groups.
