Our MOFD wards ran a 3 day event to raise awareness around reducing deconditioning in hospital. There were lots of activities, sessions provided by @CarersLeeds, LTHT Dietetics, LTHT Continence Team, @LUFC, The Happiness Program by @SocialAblty.

Day 2: It was really exciting to observe the dedication of the staff in OPAT to ensure patients don't miss their antimicrobials. The idea of using 24 hour infusion pumps to support discharge from hospital is fantastic! @LeedsHospitals @LTHT_SIM @LTHPharmacy
