It's an old technique but once you don't have enough urethra to anastomose or augment, pedicled penile skin flap-tube might be the best option left. We also did hyperbaric oxygen therapy (HBOT), which probably helped a-lot. @abalajontoto #redourethroplasty #Pedicledskinflap

Just to clarify, the pathology is a urethral stricture and not a narrowing of the prostate. Alpha-blockers won't relieve symptoms caused by a fibrosed/scarred urethra. #failedtamsulosinchallenge

One characteristic distinguishes urethral stricture from prostatitis: they respond poorly or not at all to tamsulosin or any alpha-blockers. I call this failure of the tamsulosin challenge. #urethralstricture #tamsulosinchallenge

What happens after 360° urethrovesical anastomosis distracts? Pt w/foley+transglut drain x3mo post RARP. Foley balloon sat in prostate fossa keeping BN open (CT scan). Initially pt able to void/empty, now dribbling. Cysto 2 mo after foley removal (video). Plan: Robotic BN recon.
