Special place in hell for people who launch into asking for clinical advice over the phone without telling me who they are and which team they are working with

@tomayates Absolutely. I think flexibility is the key. For some reason UK system currently assumes all trainees need same duration of training no matter what their career plans are. (Presume service provision is some of the reason for this)

@LucyCKBell I'm sure that is true Perhaps flexibility is what is needed? If aiming for mostly research and a discrete clinical role, you may want more focussed clinical training If you see yourself doing medical takes or manageing comorbid ward patients then that may not be appropriate

I am happy with the length of my clinical training and learn a lot doing out of hours work IMHO more important threats to clinical academic careers are - reduced pay on academic blocks (risks research being limited to doctors who have family help with mortgage deposits) [1/2]

@tomayates Obviously people need an adequate amount of training but a move to a competency based system (where you’re not obliged to do 60 months of SpR training no matter what) could reduce this

@tomayates I do think extended duration of clinical training has impacts though. Partic for female academic trainees who may take further TOOT for mat leave/work LTFT for childcare (I know male trainees also take parental leave and work LTFT - but this disproportionally affects females)

There is a crisis in clinical academia in the UK. We talk of the need to expand medical schools, embed research into the NHS to improve care, & the importance of a vibrant life sciences sector for the economy of the UK, & yet we seem unable to tackle the underlying issues 1/9
