I lost my dad today. He was a farmer all his life, and his insane work ethic every day to build us a better life certainly rubbed off on me and my brother. And he was a phenomenal storyteller who loved to chop it up with anyone he met, so maybe he passed that on to me too. Most of all, he was one of the best, most tolerant and most rational men I knew. I’m gonna miss him, but I’m glad we got one last Christmas with him and I’m glad I got to see the joy in his eyes as a grandfather. Tell your loved ones you love ‘em.

@BezBeyondThePen I absolutely love Portillo‘s because I can get it just about anywhere, but when I’m in the Chicago area, I always look for the Mom & Pop places. That’s where beef gets done. That’s where hotdog dogs are wrapped with their own french fries.

A couple things that puzzle me on the most recent AP Poll: Two loss Notre Dame: Ranked 13th. Those two losses: No.2 Miami and No. 4 Texas A&M. Two loss Illinois: Unranked. Their two losses: No. 1 Ohio State and No. 3 Indiana. One loss USC: No. 21. That one loss: #Illinois.

As an outpatient IM/primary care physician who is on the receiving end of hospital notes, the pattern that gets followed: copy and paste the ER cc and HPI. Keep that same note in every subsequent hospital progress note. (don't update with changes in symptoms, etc). Add a table of daily vital signs, including I/O's. copy and paste the result of every single imaging study (for some reason, lab results are not typically added - there's just something about imaging reports 🤔) For discharge summary, just do the same (again, use the ER chief complaint and the ER detailed HPI). Add a discharge "problem list" that does not need to include the final diagnosis. It seems specific diagnoses are considered a bit superfluous in this setting. Tell the patient to follow up with PCP and you're done.
