@SeanDreyer I remember an intern presenting in GI rounds his patient with “ma-LEE-na” when all the other docs, including GI, said mel-A-na. I pulled him aside and corrected him so he could stop embarrassing himself. Turns out his pronunciation was correct according to Merriam-Webster online!

@BenWThompson At my hospice IPU generally not. When working as a palli consultant, I feel it is important to explain both to the pt/family, RN and primary team the intention of the orders. I tend to use it more for patients who seem reluctant to take meds but benefiting greatly from them.

Outpatient pain management orders and inpatient pain management orders should be different. PRN medications can be taken within seconds of the request at home, but can take an hour to receive in the hospital. I've started scheduling some "PRN" meds (case dependent) 1/2

@BenWThompson I’ve had a similar practice for some time now Ben. I will explain it to my patients that the nurse will bring you your medication every __ hours and you can decide to “opt out” if you don’t want your medication at that time. Rather than having to “opt in” and wait for an hour….

2/2 on the longer side of the PRN (Q6 if I'd write Q4-6 at home) and instructing patients that they can decline the medication when it's brought. Am I crazy??#PatientCenteredCare #hapc #medtwitter

@SoniaMKhunkhun Probably THE biggest public misperception about hospice. So often patients and families ask me day 1 or 2 of hospice admission- when is the morphine gtt gonna start? We use the LEAST amount of medication you need to be comfortable and live the best you can as long as you can!

The practice of morphine drips in every end of life patient needs to stop. We need to tailor care of #endoflife #hospice patients toward what their needs are. And not every patient needs a morphine drip. In fact, this “one box fits all” approach amplifies fear. #hapc
