@jim_crawfurd @medicalmodelbri @nhsswipecard If I'm about to board a plane and the pilot is sick, I would not appreciate a senior pilot giving me a non-pilot and saying it's better than a gap. I would not get on the plane. How do we safeguard patients from this effect who are after emergent care?

There needs to be an open and honest discussion with the public who on the whole have no idea this is going on & would likely find it unacceptable unsafe deceptive not informed consent Poor value & area variable for tax/NI £

The systematic undermining of general practice has consequences that you can't even imagine. When #1 was 2 weeks old, he developed a fever, was grey and floppy with a bulging fontanelle. I called the GP OOH and he was seen within half an hour of my call. IM benzylpenicillin and a week in hospital. That OOH GP saved his life, no question. He has just been offered a place to do an MSc in mathematical and theoretical physics at Oxford - unbelievably competitive. Do I think today's 111 would have achieved the same? Not a chance.

As I reflected on the Covid Enquiry with @NickFerrariLBC on @LBC I was reminded of the loss We need to take healthcare/NHS away from Politicians The politicians need to be told what is necessary & it’s their job to make it possible with funding The opposite is happening

‘I’d do it all again’ Matt Hancock As a GP practice we lost a third of Nursing home residents in the first wave due to the decisions made by leaders to discharge infected patients to homes It was done without mitigation of the risks & limited/poor PPE

HF nurse - I’ve seen this patient for heart failure and made no changes to what the GP did. Discharge back to GP for rapid heart rate to “consider rate control” WHAT’S THE BLEEDING POINT OF YOU?! This bureaucracy is maddening. FFS!

@DrSteveTaylor @wesstreeting The aim is to manipulate waiting list figures but ALSO to set up/ normalise a system to cater for a planned primary care service staffed by non doctor roles who will need this support when actual GPs who don’t need this support are no longer employed in primary care

Advice and Guidance is going to move NHS waiting lists to the community ‘From Hospital to Community’ that’s what @wesstreeting meant It doesn’t reduce referrals the data proves this there are more A&Gs but no fewer referrals It will remove patient choice & undervalues GPs

In the Nursing and Midwifery Council's own words - Advanced Practice poses a risk to the public, and they will only be *beginning* to develop standards of proficiency in 2027. Meanwhile, they on the same rotas as doctors.

@medicalmodelbri Huge concerns around the PA role, most importantly patient safety risk, confirmed by the Leng Review So sending any PA ,let alone an inexperienced one, out to patients who are often the most unwell/complex is a very poor service & unfair to pts and a recipe for disaster

@DrLKVaughan @jim_crawfurd @TheBreakster @medicalmodelbri It's unfathomable that there isn't a period of 'back to school' for a good theoretical grounding proven by exam BEFORE starting pt contact As worrying- countless examples (see @kciisc) of tACPs taking years 5/7/12 - to complete/pass Msc but working as ACP Does anyone fail?

The next, seeing undifferentiated pts without so much as an introductory lecture or regulatory safeguards. This is legitimately insane. 3/

@kcisc This person is not obtaining informed consent from patients & encouraging others to do likewise. Huge red flag PAs always have to be supervised by a named senior Dr No way round it Patients have the right to know who is supervising, delegating & responsible for their care/safety

@kcisc Note also the borrowing of terminology associated with Dr training inc ‘rotations’ to feign gravitas & legitimacy 13+ specialties in 7 months= just days in each Hardly touched the surface of anything Shows just how shallow & therefore dangerous the PA training/role is

A good summary of the issues surrounding physician’s associates in the NHS. My concern (always) is the NHS trying to cut corners/ do things on the cheap rarely ends well for patients. Medicine is a complex, risky field & years of training are needed. 🙏 thetimes.com/article/edad92…

@valhumphreys51 @parthaskar Really well captured Val. That theme of patients being kept in the dark while processes happen in the background is so key and exactly what's so concerning. Politicians and regulators doing dodgy deals while kept firmly safe and insulated from any consequences.
