49 year-old woman with "optic neuritis" in both eyes and an MRI with white matter lesions (see image). Multiple Sclerosis? No! Panel J is MR spectroscopy showing inverted double peak of lactic acid. Diagnosis? I have two patients with this who were misdiagnosed with MS!
