Rachel Pilgrim did this work as part of a research internship which HEE supported for practising pharmacists when I was at U of Bath. She's now doing a PhD building on the work in this paper. A great example of pharmacy practitioners doing research to improve practice.

Using generalisability theory, we show that the new system is as valid and reliable as one of the leading existing measures. With recent developments in the field, there are things we'd now like to try that might improve it further. (3/3)

No one had really tried this before, but lots of people scoring the potential harm of errors consider that various outcomes are possible, so are unsure whether to score the most severe harm, or the most likely etc. This system addresses that problem. (2/3)
