The day the Affordable Care Act passed was one of my proudest moments as president, because it meant that millions of Americans would have access to health care, some for the first time. The ACA also prevented insurance companies from denying people with pre-existing conditions coverage, allowed young people under the age of 26 to remain on their parents’ plan, expanded Medicaid, and so much more. But the ACA was always meant to be a first step. We still have to do more to expand access and make health care more affordable for everyone.

The hospital makes 2 to 2.5x more money from a C-section than a vaginal birth. And if the baby ends up in the NICU, it's even more revenue. NICU admissions have nearly doubled in the last 15 to 20 years, from just over 5% to almost 10% of all births, according to the CDC. It's worth asking how we got here. Part of the answer is expediency. A scheduled C-section at 7:30am is done by 8:20. That kind of predictability is easier to build a hospital system around than individualized care for each woman's labor. The other part is reimbursement. If we wanted to lower the C-section rate, we could change how we pay for births. If insurance paid more for vaginal deliveries than cesareans, you'd be watching the incentives shift. The doctor may not pocket the difference, but the hospital does, and the doctor saves time either way. Pregnancy is the only reason you go to a hospital for something that is a completely normal physiologic function. That context matters when we talk about how birth decisions actually get made.
