A nurse practitioner (non-palliative care) did a wonderful job in goals care conversation. I gave her the positive feedback. She said this is what I taught her. I think I can get drunken today.

@georgetolisjr Sagittal view can be used to show the length of the aorta between where the RPA crosses posteriorly (this point is almost always above STJ), and where the innominate vein crosses anteriorly, a reasonable surrogate of “usable” aorta.

I still cannot predict from a pre operative chest CT if the aorta is elongated (ie heart is low) or short, stubby and hard to cannulate/do proximals (heart pushed toward the neck) because of lack of real estate. Can have implications RIMA reach. Does anyone have any hints?

The verb “diuresing” is often erroneously used to describe any source of bodily output. No one “diureses “ from their chest tubes. Their output may affect the patient’s daily Is & Os but what is coming out is serous fluid, not urine.

@Awesomecloss @Caulimovirus Mine was ping pong balls. As much of a curiosity as they were, I never liked seeing those old CXRs of ping pong balls in the chest. Subconsciously maybe because they made me think of how much fun I’d be having playing ping pong instead of taking the board exam with the photo.

@Caulimovirus Can’t believe no one has said Tessalon Perles. We have nothing for your cough and we need to just admit it.

This is still the culmination of every year, a time to forget about CABGs, root enlargement definitions and Prolene and by far the most fulfilling family outing of the year. Merry Christmas to everyone!

@MNateShyamalan @thats_bone Since Sears went out of business it’s the only place you can get lingerie and tires in one trip. Also the only place ever where you can get croissants and a telescope.

@georgetolisjr @MGHSurgery Thank you George for your help with this case but more importantly for the many hours you spent training me years ago. Your ongoing dedication to resident/fellow education remains unparalleled and should be recognized way more than it is and has been.

I had the unique privilege of assisting Dr. Kwon @mike5966 a former trainee of mine from my @MGHSurgery days with a complex adult congenital case, which he executed flawlessly, reducing my role into a mere cheerleader. There is no greater satisfaction for a surgical educator.

Decided to moonlight on pediatric cardiology after 2 years of being on the adult side…my thoughts: 1) 👶s are so adorable 2) NICU nurses surprisingly love me still 3) everyone is so confused by my return 4) I’m still horrible at swaddling

@TsengJennifer @BUMedicine @AlikFarber @The_BMC @BMCSurgery Work/life balance is important but take a long view of it. Sometimes the balance is over months and years, not day-to-day. Sometimes work dominates - especially as a trainee - and that’s ok. It’s easy to make work/life balance a source of stress.
