10-year-old Sara Sharif was found dead in her home in Surrey last year. She was murdered by her father and stepmother. Children's Commissioner Dame Rachel de Souza, is demanding urgent reforms, saying Sara was "failed in the starkest terms", adding there should be "no more reviews, no more strategies, no more debate". @richardgaisford reports.
