This study of 77 females and males in Uruguay takes a deep dive into the uriobiome and compares bacterial diversity, intracellular bacterial communities and people with and without urinary symptoms. The study concurs with many previous studies that highlight the incompetence of traditional culture based diagnostics and the need for new microbiome based testing and treatments. This UTI research used nanogels combined with a special peptide to push gentamicin inside infected bladder wall cells to clear the infection. The researchers say nanogel delivers about 36% more antibiotic inside cells compared to standard antibiotic delivery methods. No, we are not ahead of the game, we are 90 years behind. The microscopic method was developed in 1928 but was phased out in the 1970s. The replacement central laboratory methods and dipsticks were introduced without proper validation. A terrible blunder.

“The bladder itself is generally considered a sterile environment” - Science shows the normal bladder the antithesis of sterile. Interpret that with the lense of Darwinian evolution and the entire urine culture edifice crashes to a pile of rubble. We must rethink UTI diagnosis.
