I would specifically investigate stenoses of the transverse sinus or sigmoid sinus as a potential cause of intracranial hypertension. We have a series of at least 15 patients with confirmed venous sinus stenosis who were treated with venous sinus stenting. Lecanemab and long term Alzheimer’s: are we finally bending the curve? Low baseline tau seems to be important. Amyloid refers to sticky protein clumps in the brain that are believed to contribute to Alzheimer's disease. Christopher van Dyck and colleagues describe in an abstract from this years American Academy of Neurology meeting the most recent findings from the Clarity AD open label extension study examining up to 48 months of treatment w/ lecanemab in early Alzheimer’s disease. Key points: - Lecanemab treatment showed continued slowing of cognitive and functional decline through 48 months compared to matched controls. - Treatment differences increased over time suggesting a possible cumulative or durable disease modifying effect. - A meaningful proportion of participants showed no decline or even improvement especially in those w/ low baseline tau. My take: This is important as longitudinal signals in Alzheimer’s disease matter. The idea that treatment effects may grow over time is provocative and raises the possibility that early and sustained intervention could meaningfully alter trajectory. At the same time we must remain cautious as these are open label extension data, and longer term real world validation will also matter. Here are 5 points that resonated w/ me: 1- Early treatment may be key to maximizing benefit in Alzheimer’s disease. 2- Slowing decline over years rather than months is what folks and families care about most. 3- The subgroup w/ low tau may represent a window where intervention is most effective. 4- Safety signals appear stable over time which is reassuring for ongoing use. 5- The future may include combining amyloid therapies w/ other approaches to further shift the disease course. index.mirasmart.com/AAN2026/PDFfil… @ParkinsonDotOrg #parkinson #alzheimer @alzassociation

Hikaru Nakamura appreciation post. He is getting way too much disrespect right now. I know he has always had haters because of how directly he speaks and because of some of his actions over the years. But I also think qualifying for the Candidates through those Mickey Mouse events rubbed a lot of chess fans the wrong way, and he is paying the price for that now. Honestly, I think if he had played Grand Swiss and a few more serious classical events in 2025, he would have come into this tournament in much better shape. I also think part of the issue is motivation. It feels like he wanted to play the Candidates mainly because it gave him a path to the World Championship, not because he loves classical chess at this point. And when you already seem half-detached from classical, it is very hard to show up to the toughest event in the world and perform at your best. Because he is having a bad event, everything he says is now being judged through the lens of losing. My feed is full of criticism. When Hikaru criticized the anti-cheating measures, I think he had a point. When he criticized his own team for missing 12...0-0, I think he had a point there, too. But once a player is losing, people stop listening to the argument and start attacking the person. That is exactly what is happening here. I am not even a big Hikaru fan. But people should remember who he is. He is a legendary player. His online chess over the last 20 years is unmatched. In classical chess, he crossed 2800 twice. Only 16 players in history have ever reached that level, and not all of them made it back after dropping below it. Hikaru did. Around 2020, when he became more of a streamer, a lot of people stopped taking him seriously as a top player. Then he came back in 2022 and proved again that he is exceptional. And beyond that, his work as a streamer has been huge for chess. He brought a massive number of new people into the game. He has also consistently brought attention to issues inside the chess world and forced conversations that others would rather avoid. You do not have to like Hikaru. I am not saying that. But the amount of disrespect he gets is absurd. He is one of the most important chess figures of his era.

There are 13 games in the Lichess database in the position where Hikaru blamed his seconds. The most common move (7 out of 13) was Ne4, the best move. Hiakru played h4. It seems like we're at the point where top Chess GMs need to hire analytical experts / software engineers to be part of their team. There's no reason a human should be at fault for a move like this not being in the file. This is an easy problem to solve with software.

Aber der Schaden kumuliert. Damit sinkt die Nachfrage. Infolge steigt die Arbeitslosigkeit und das verfügbare Einkommen sinkt dann aus zwei Richtungen. Ein Teufelskreislauf aus dem es kein entrinnen gibt. Im Kampf der Deflation mit der Inflation gibt es keinen Normalzustand mehr. Es bleibt abzuwarten wie der Michel darauf reagiert. Vorbereitet scheint er nicht zu sein.

Manche fragen sich hier warum es keinen Aufstand der Bevölkerung gibt. Tja. Einfache Antwort: es geht den Menschen gut. Deutschland gibt für Migration, Bürgergeld und Entwicklungshilfe rund 145 Mrd. EUR aus jedes Jahr. Das sind knapp 300 EUR für jeden Haushalt. Jeden Monat. Stört niemanden. Der durchschnittliche Autofahrer fährt 13.000 Kilometer im Jahr. Selbst bei mehr als 7 Liter Verbrauch bedeutet das 80 Liter im Monat und Mehrkosten von nicht einmal 40 EUR im Monat. Deswegen soll es einen Aufstand geben? Das ist schlicht lächerlich. Eine "normale" kleine Familie muß sich darauf einstellen, dass die monatliche Mehrbelastung in den nächsten 3 Jahren nicht 40 EUR sondern locker 500 EUR betragen wird. Und auch dann wird es keinen Aufstand geben. Und so lange das so ist darf man gerne hier auf X jeden Tag schreiben "Ich will das aber nicht!". Das ändert aber an der Situation gar nichts.
