This might seem like 'nothing new' but the continuity of acute NHS care at home to act as a bridge to well titrated Local Authority care is contrary to many historic paradigms and breaks down barriers. System thinking with aligned ownership of solution and risk.

Today, Trauma & Orthopaedics opened their new four bedded Frailty Admission & Discharge Unit (FADU) at the QMC. The aim of the new Unit is to improve the current pathway, as well as improving patient care and experience. It's open from 7am – 7.30pm, 7 days a week.

@IanTyrrell5 Couldn't agree more, as a community nurse it's so frustrating not being able to access therapy support for weeks. The patients I work with don't have the time to waste waiting

I still wish for the complete admission avoidance package! Yes I want utopia but we need proper access to community services who have the ability to respond within hours not weeks or days! Front door teams are only as good as the services they can access!
